Pre-Service Application Process for Spring 2013

The following items must be completed and submitted by April 15, 2013:

• Application Form (MS Word)
• Release Statement (MS Word)
• Physician's Statement (PDF)
• Autobiography (MS Word)
• EMPCO Reading/Writing Test results
• MSCTC Physical Fitness Test results

 If you are not a current Delta College student, you must complete a Delta College admission application.

Course Fee:    In-District (Bay, Midland or Saginaw Counties):    $1,216.00 per student

                      Out-District (All Other Michigan Counties):    $1,576.00 per student

                      Out-State:    $2,052.00 per student
