Exam Coversheets

Individual exams brought to the Academic Testing Center must have a coversheet attached to it.   All exams taken in the ATC are entered into a database which is cross referenced with Records and Registration to ensure only those students enrolled in specific courses and section numbers have access to them.  Coversheets also provide specific instructions as to how and when each exam is to be adminstered to students by the ATC staff.

The three (3) forms used by the ATC are listed below and can be downloaded at any time.  They have been saved as .doc files for easy access. 

• Form 1:  Make Up Exam Coversheet 
• Form 2:  Regular Exam Coversheet
• Form 3:  Recycleable Exam Coversheet -- use when the faculty member does NOT want students to write directly on exam
