    Appeal Form

Academic Progress Appeal forms received by Monday, January 4, 2010, will be reviewed for the Winter 2010 semester.

Appeals received after this date will be considered appeals for the Spring 2010 semester, unless students have registered and paid for Winter 2010 courses and are in attendance.

Students who have been terminated from further financial aid for failure to meet Standards of Academic Progress for Financial Aid Recipients have the right to appeal. All appeals must be submitted in writing on the Satisfactory Academic Progress Appeal Form to the Financial Aid Office.

    Verification Worksheet

The Federal Government routinely selects some financial aid applications for a more in-depth review process called “Verification.” In this process, Delta College is required to compare the information from a student’s financial aid application with copies of the student’s/parent’s signed Federal tax returns, W-2s and/or other financial documentation.

To comply with this requirement you must select/print/complete and submit the Verification Worksheet to the Financial Aid Office.  Be sure to select the Verification Worksheet which corresponds to the academic  year for which you are requesting financial aid.

    Student Loan Request Form

The Federal Family Educational Loan Program (referred to as FFELP) provides low-interest educational loans made by private lending institutions or non-profit agencies to the student or to the student’s parent.  These loans are insured by the Michigan Guaranty Agency and reinsured by the federal government.

    Scholarship Applications

Scholarship Awards are usually based on achievement or talent and are a form of financial aid that usually does not have to be paid back. Seek out campus resources: Delta College’s Financial Aid Office should always be the first place you go to ask about scholarships. But, professors and academic advisors may also know about grant opportunities available to students.

    Parental Non-Participation Form

As a dependent student, you are required to provide parental information on the FASFA.  Although we were able to submit the FASFA, your application is considered incomplete because no Expected Family Contribution (EFC) could be calculated by the Federal Government.  To be considered for Federal Title IV financial aid, you must either update your FASFA by providing required parental information at www.fafsa.ed.gov or have the following form completed by your parent(s) and return the signed form to the Financial Aid Office to be considered only for a Federal Stafford Unsubsidized loan, with the maximum amount of $2000 per academic year.

Non-Credit Board of Trustees Grant Application

Financial assistance may be available for students enrolling for short-term training program courses as a part of the Lifelong Learning Office.  The application should be completed at least two weeks prior to expected enrollment.
